We Accept a Majority of PPO Insurance Plans

We accept Aetna, Blue Cross / Blue Shield Illinois, Cigna, Guardian, MetLife, United Healthcare, and Delta Dental. 

Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Charges are payable at the time of treatment. For your convenience, we accept cash, checks, Visa, Mastercard and American Express.

We also have other payment options for patients who do not have dental insurance or are worried about what their insurance plan might not cover. These include:

We strive to find a payment option suitable to your needs; we work with third-party financing institutions including SunBit and Proceed Finance.

$144/Year per patient

Our Image Card Membership program is designed to help patients take care of their oral health with important preventative treatments. It includes the following benefits: 

  • Two Dental Exams Per Year
  • Two Sets of Free Dental X-Rays
  • A 30% discount on all other general dentistry procedures 
  • No Waiting Periods
  • No Maximums

Please note, that our membership program is non-refundable and cannot be combined with any of our other dental specials. Dental treatments are paid for on the date of service. Our membership is only applicable at the office where it was purchased, it doe snot cover referrals to specialty offices, or specialty procedures like oral surgery, implants, or orthodontics. 

If special arrangements are needed, please talk to our office manager prior to receiving service.

Additionally, we take in-network with many dental insurance providers. We also take most PPO plans. Please ask our office manager during your visit for more information insurance coverage and our payment plans.

We will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days.
